The primary purpose of this position is to coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of facility residents in accordance with current federal, state and local standards, guidelines and regulations governing the facility and to monitor the dietary department so as to be maintained in a clean, safe and sanitary manner as may be directed by the executive director, to assure that quality nutritional services are provided on a daily basis.

Essential Job Functions Duties and Responsibilities

Clinical Functions

  1. Assesses and interviews all new admissions. Maintain compliance with regulatory guidelines (quarterly or 30 day) for dietary needs and progress.
  2. Reviews resident weights and weight trends.
  3. Follows skin breakdown and initiates interventions as appropriate
  4. Completes MDS information for Medicare compliance
  5. Addresses, consults as generated from medical staff
  6. Participates in annual, and any other, state or Federal surveys as part of the interdisciplinary team
  7. Provides nutrition education to residents and/or family members

Care Plan and Assessment

  1. Interview residents or family members to obtain diet history. Participate in maintaining records of the resident's food preferences. Involve the resident/family in planning nutrition education and goals for the resident. Assist in developing diet instruction materials for individual residents.
  2. Review clinical notes to determine if the care plan is being followed.
  3. Develop a written plan of care that identifies the clinical needs/problems of the resident and the goals to be accomplished for each assessed need identified with the appropriate interventions.
  4. Review and revise care plans and assessments as necessary, but at least quarterly.
  5. Attend care plan conferences as assigned. Complete Resident Assessment Instruction as part of the MDS process as assigned.
  6. Complete routine documentation on assigned residents in the medical record following facility/departmental policy guidelines and governing body standards.
  7. Participate in interdisciplinary team committees or meetings as assigned including but not limited to weight monitoring, pressure ulcer/wound monitoring, or other nutrition related clinical topics.

Required Knowledge, Skills and Abilities

Must possess the ability to make independent decisions, follow instructions, and accept constructive criticism.  Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.  Must be able to work with ill, disabled, elderly, emotionally upset, and potentially hostile people within the facility. Must be able to speak, write and understand English in a manner that is sufficient for effective communication with supervisors, employees, residents, and families.

Education and Experience

Minimum of a Bachelor's Degree in Dietetics from an accredited college or university.

Prefer experience in a supervisory capacity in a subacute, long-term care facility, or other related industry.


Registered by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.

License in Nutrition/Dietetics to practice as a dietitian as required by the applicable state.

Maintain Registered Dietitian/Nutritionist (RDN) status including continued education per Commission on Dietetic Registration standards.
